POSITION SUMMARY

Clinical pharmacy services in the outpatient setting are an area of expanded interest nationwide, and particularly in Northeast Ohio. The introduction of the Collaborative Practice Agreement (CPA) in Ohio has served to elevate the level of practice and encourage greater involvement of pharmacists in the outpatient provision of patient care and medication management.

The focus of this position will be on providing comprehensive pharmaceutical care, disease management and specialty pharmacy support in the ambulatory care setting to multiple sclerosis patients seen by the University Hospitals Neurological Institute. This individual will ensure continuity of care as the patient transitions from the acute care to the ambulatory care setting, while continuing to pursue best practices in this area. This position will also be responsible for specialty pharmacy support in the ambulatory care setting to adult patients seen at these clinics. This individual will also be responsible for providing experiential instruction to Doctor of Pharmacy Students and University Hospitals residents in support of our goals for academic excellence.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The primary job responsibilities include ensuring efficient medication use by serving distributional functions within the clinic and referring patients to University Hospitals Specialty Pharmacy, as well as providing comprehensive pharmaceutical care for multiple sclerosis patients seen in the University Hospitals Neurological Institute through innovative clinical services.

Clinical responsibilities (65%)

• Building a CPA clinic that collaborates with physicians to see patients and bill for services
• Serving as a liaison between the Department of Pharmacy, Specialty Pharmacy, and the University Hospitals Neurological Institute to assure all activities are coordinated and consistent
• Working closely with clinic physicians to determine appropriate medication plans for patients
• Providing longitudinal dosing and monitoring of appropriate regimens
• Responsible for all medication management goals set forth by the institution including medication use evaluations and reconciliation
• Educating patients and families about their treatment plans and subsequent medication management
• Educating staff on guideline updates and practice advancements associated with this patient population
• Managing toxicities and adverse effects caused by medication regimens for all patients seen in the clinic
• Reviewing all orders written by clinic physicians prior to being sent to the pharmacy and if inaccurate, the clinical specialist will collaborate with physicians to have orders rewritten prior to sending them to the pharmacy
• Managing transitions of care for patients from the acute to ambulatory care setting and vice versa

Distributional Responsibilities (25%)

• Identifying and enrolling eligible patients for specialty pharmacy services from the UH Specialty Pharmacy, and serving as the primary contact for the University Hospitals Neurological Institute
• Responsible for retail distributional operations of the University Hospitals Neurological Institute, that may include facilitating refill requests, insurance adjudication, pharmacy restrictions, and coordination of authorizations/medication assistance
• Conducting follow-up phone calls to the patients to support the Specialty Pharmacy Clinical Program with patient contact at routine and scheduled intervals
• Coordinating with the UH Specialty Pharmacy to verify patients’ insurance requirements to ensure timely processing of all prior authorizations and to provide direction for available financial assistance resources

Educational / Administrative / Research Responsibilities (10%)

• Educating students from the statewide schools of pharmacy, PGY1, and PGY2 residents from UH Health Care System on their experiential rotations
• Obtaining an academic appointment with the Northeast Ohio Medical University School of Pharmacy is available, commensurate with experience
• Serving as a standing committee member on a select group of committees within UH Health System
• Conducting research in collaboration with primary investigator in research studies

Qualifications

QUALIFICATIONS & REQUIRED EXPERIENCE

• Doctor of Pharmacy Degree from an accredited college or university or equivalent experience required
• Completion of a Pharmacy Practice (PGY1) and/or (PGY2) Residency or equivalent experience preferred
• Eligibility to enter a Collaborative Practice Agreement in the State of Ohio
• Licensure or license-eligible as a pharmacist in the State of Ohio

OTHER SKILLS AND ATTRIBUTES

• Extensive clinical skills and pharmacotherapy knowledge base in MS, neurology or equivalent work experience
• Operational and distributive experience in preparation of hazardous drugs
• Ability to work in a team environment
• Strong oral and written communication, organizational, and prioritization skills
• Ability to design and implement projects, collect and interpret data accurately, and prepare project outcomes manuscripts for submission in collaboration with colleagues
