POSITION SUMMARY
Clinical pharmacy services in the outpatient setting are an area of expanded interest nationwide, and particularly in Northeast Ohio. The introduction of the Collaborative Practice Agreement (CPA) in Ohio has served to elevate the level of practice and encourage greater involvement of pharmacists in the outpatient provision of patient care and medication management.
The focus of this position will be on providing comprehensive pharmaceutical care, disease management and specialty pharmacy support in the ambulatory care setting to multiple sclerosis patients seen by the University Hospitals Neurological Institute. This individual will ensure continuity of care as the patient transitions from the acute care to the ambulatory care setting, while continuing to pursue best practices in this area. This position will also be responsible for specialty pharmacy support in the ambulatory care setting to adult patients seen at these clinics. This individual will also be responsible for providing experiential instruction to Doctor of Pharmacy Students and University Hospitals residents in support of our goals for academic excellence.
ESSENTIAL DUTIES AND RESPONSIBILITIES
The primary job responsibilities include ensuring efficient medication use by serving distributional functions within the clinic and referring patients to University Hospitals Specialty Pharmacy, as well as providing comprehensive pharmaceutical care for multiple sclerosis patients seen in the University Hospitals Neurological Institute through innovative clinical services.
