The Navigator position will provide non-clerical and non-clinical support to patients and their family members to navigate appointments, services, resources, and schedules related to the care provided within the Division of Infectious Diseases for the multidisciplinary endocarditis team (MDET). The MDET is a multidisciplinary endeavor between the divisions of infectious diseases, cardiac surgery and addiction medicine that is designed to improve the in-hospital and long-term care of patients with endocarditis and other cardiac infections. The Navigator will be assisting in coordinating the transitions of care and outpatient follow-up for all patients seen by the Infectious Diseases MDET providers. They will meet patients in the hospital prior to discharge, assist in arranging follow-up appointments and coordinating appointments with other specialties. They will address outpatient phone calls and clinical questions and triage these to the appropriate physician. They will assist in ordering and scheduling testing and blood work. They will be expected to develop a longitudinal relationship with the patients so that they become an integral part of their care team.
Given the COVID-19 pandemic, and the need to protect patient, employee, and visitor safety, UK HealthCare is requiring all employees to comply with the COVID-19 vaccination policy prior to starting employment. Therefore, as of September 15, 2021, UK HealthCare requires all employees, including new hires prior to their first day of employment, to submit appropriate documentation showing, i) completion of the first vaccination against COVID-19, or ii) completion of the full vaccination series against COVID-19 (i.e., two weeks past completing their complete COVID-19 vaccination series), or iii) approved exemption (medical contraindications or a sincerely held religious belief) through the formal vaccine exemption process from the COVID-19 vaccination requirement. For those employees who have completed the first vaccination against COVID-19 prior to their first day of employment, the second vaccination (if applicable) must be completed following the vaccination recommended schedule to be considered fully vaccinated.
